Senior LinkAge Line™
Minnesota is evolving a
unique version of the resource center concept -- creating a virtual and
human resource network -- known as the MinnesotaHelp Long-term Care Choices -- to
provide people with resources and guidance in long-term care planning and
decision-making.
The ADRC’s focus on strengthening opportunities for
informed care choices and streamlined access to resources provides the
Minnesota Memory Care Initiative an important opportunity to embed memory
care identification, treatment and support strategies within Minnesota’s
primary system of service access. To accomplish this, the Alzheimer’s
Demonstration is collaborating with the ADRC on the following initiatives
that feature Alzheimer’s disease identification and support services:

Data integrity for

MinnesotaHelp Long-term Care Choices,
a web-based decision tool for seniors, families, and
