The Minnesota Alzheimer’s Demonstration Project

 

The Minnesota “Working Together” Alzheimer’s Demonstration project is sponsored by the Minnesota Board on Aging and funded in part through the Administration on Aging’s Alzheimer’s Disease Grants to States.  The Alzheimer's Association Minnesota North Dakota Chapter partners with us to bring you this program.

 

Early diagnosis and treatment of memory loss has tremendous potential.  This potential lies in treatment and health care planning that is coordinated with community-based supports for both the person suffering from memory loss and their caregivers.  

 

This Demonstration Project uses a pathways approach to work within medical and community services to enhance the delivery of care to people suffering from memory loss and their families.  This Demonstration is based on the best practices of the Chronic Care Networks for Alzheimer’s Disease (CCN-AD) project. This approach is designed to ease barriers to accessing and coordinating community care and medical services.  It is our goal, using a “no wrong door approach”; that no matter where a person with memory loss or their family caregivers access services:

1. Possible memory loss will be identified

2. An initial memory loss assessment will be conducted by a medical provider in cooperation with community partners

3. Care management will be provided that insures ongoing support that is customized to fit local resources.

4. Training and support that allows the development of the skills and knowledge to provide the best care possible

 

This Demonstration is being piloted in four rural areas of Minnesota and one urban minority community.  It is the goal of this Demonstration that pathways, strategies and tools for working with medical and community partners will be adopted by partnering medical and community organizations throughout Minnesota. 

 

Our Goal:

Collaborating memory loss capable medical care and community services working with people with memory loss and their caregivers’ support independent living throughout Minnesota.
