Replication Tools
The primary purpose of
the Minnesota Memory Care Initiative and this web site is to encourage and
facilitate the replication of the strategies developed by the pilot
projects and the CCN-AD project.
It is our goal that
every clinic, social service agency, Faith-in-Action or Living at
Home/Block Nurse Project have the capacity to…
… recognize
the signs and symptoms of possible significant memory loss,
….have the connections
within their communities to ensure that an assessment leading to a
diagnosis and treatment is made,
…provide care
management through a social worker or nurse to connect the
individual’s community care plan with their medical treatment plan
… train and support
family caregivers to develop the skills and knowledge to provide the best care
possible
Each of the pilot
projects whether funded through the “Working Together” Alzheimer’s
Demonstration Project or state funded Community Service/Service
Development grants is developing tools and strategies to address these
issues. We want all communities to have the opportunity to develop these
skills and embed them in their systems, regardless of whether they are
able to participate in a pilot.
We are making
available to you all of the tools, materials and protocols that they have
developed so that you can adapt them for use within your system. This
approach of systems change within existing community programs should be
more feasible and sustainable--without new funding--if networks use
resources contained on this site.
The Coordinator of the

The Alzheimer's
Association offers training, information, and guidance. The Alzheimer's

in your efforts to embed memory loss capacity within your organization and

The Minnesota
Alzheimer’s Demonstration Project has developed additional replication
materials in several areas that are available for your use as well.
Attached are materials and tools for volunteer, in home and group
respite and Memory Assessment Clinics. In addition we have
developed a replication manual to create Joy Kits, kits that are
used to spend quality one-on-one time with someone struggling with memory
loss.
